Reception to Year 2 Children
Today in your child(ren's) book bag you will find a letter and consent form regarding the influenza immunisation. Please read the information sheet and complete the attached consent form (one for each child) and return to the class Teacher as soon as possible but no later than this Friday 9th October.
It is important to complete and return the form even if the recommended vaccination is refused. Any queries please phone the school office on 01442 211963. Thank you.
